I am having issues with understanding the solution for this exercise
If I follow the instructions given by the codebook, the final state contains the term +cos(theta/2)sin(phi/2)|0110>, but the term should be -cos(theta/2)sin(phi/2)|0110>. The sign is flipped, and I cannot figure out why. If anyone has insight as to why, help would be appreciated.

It seems like you have found a mistake 
I tried following the instructions as well, and you are right, there is no way in which you can get that minus sign. The fix is simple, we can still get the correct state if the second Givens rotation is around -\phi instead of \phi. Do you agree with me?
I will implement this fix in the Codebook ASAP.

I apologize for getting back so late. I agree with this, it is what I got. I think there are a few other minor errors as well in other parts of the VQA sections. I will take a look and get back to you.
